General Information
The FORMS II Lite software is a flexible and easy-to-use, stand-alone, Windows-based application that simplifies and accelerates the sample documentation process. FORMS II Lite reduces the generation of hand written documents by nearly 70 percent. Specifically, FORMS II Lite:
- Generates sample labels, bottle tags, and Chain of Custody (COC) forms;
- Tracks samples from the field to the laboratory;
- Facilitates electronic capture of sample information into databases; and
- Exports data electronically as .xml, .dbf, or .txt files.
Minimum System Requirements
Recommended minimum configuration requirements for the user's system are as follows:
- Windows 98, NT, ME, 2000, or XP;
- Microsoft Internet Explorer 4.0 or higher;
- 200 MHz Pentium processor;
- 32MB RAM (64MB is recommended);
- 35MB available hard disk space;
- 2X CD-ROM drive;
- 800x600 resolution using small fonts; and
- Mouse interface, printer, and remote power sources for field use.
FORMS II Lite Training
FORMS II Lite Training is provided by the Analytical Services Branch (ASB) for Superfund samplers and contractors interested in using FORMS II Lite to document their sampling events. A full training session runs one day and includes lectures and hands-on exercises. Clients are encouraged to provide their sampling scenario and documentation so that the training can be customized for specific needs.

The FORMS II Lite Version 5.1 installation CDs now include a Computer-Based Training (CBT) course that users may use to learn the basics of the software. The CBT walks the users through the software at their own pace, with quizzes at the end of each section, and a final test at the end of the course.
FORMS II Lite Web-Based Training!
Federal, Tribal, State, and local government personnel may access FORMS II Lite Training on the Web anytime and at no cost. The course is hosted by the EPA's Emergency Response Training Program (ERTP) virtual university. Please use the following instructions to access the FORMS II Lite online training. Non-governmental employees may contact Eric Reynolds at (703) 603-9928 or via email at reynolds.eric@epa.gov for training materials.
Step 1
New Users Instructions
If you are new to the EPA's Emergency Response Virtual University you will need to first register with the ERTP virtual university.
Registration: Access the ERTPVU registration page by visiting the ERTPVU login page and selecting "Register" 'from the menu at the top of the page. To begin registration, type a Login ID and Password. You will enter these each time you access the site. Your Login ID and Password should consist of at least four letters and/or numbers. Your Password is case-sensitive. Click the Submit button.
Select Organization: Either identify your organization by selecting it from the list below and clicking Select, or click Expand to view divisions within an organization. If you are a contractor, select "Other".
User Profile: As part of the registration process, you need to provide some general information. Please fill in the fields below and click the Submit button. All required fields are marked with *.
Successfully Completed: You are now a registered user of the ERTP Virtual University. Select Next to go to the Campus Map (Step 2). You can always receive navigation help through the Help function on the toolbar from any location within the site.
