Subject: Information Required when Ordering Goods and Services from Federal Agencies using the OPAC System

Reference: OFM 92-02

The attached form must be completed by USGS and attached to all transaction documents (purchase orders, letter of authority, interagency agreements, etc.*) when placing orders with other federal agencies that will bill the USGS through the On-line Payment and Collection (OPAC) system. This information is required in order for OPAC charges to be applied to the correct accounting information (USGS agreement and account numbers) and to liquidate the proper obligations.

Please ensure that all the information provided on this form is complete and accurate. If multiple accounts and multiple object classes are to be charged for a particular agreement, please indicate "Multiple" on the USGS Account Number to be Charged line and on the USGS Object Class to be Charged line. When the bill comes in for a "multiple" account or object class, the Office of Financial Management will work with the USGS contact to determine which accounts and object classes to charge.

At the time of the agreement, the charging agency should be informed that this information must be included on any bill submitted to the USGS via OPAC. If the following information is not supplied, a chargeback may occur because of insufficient and untimely data.

* Applies to all documents except for GPO print orders, GSA work authorizations, and GSA rent.
