Nominations

The Governor´s Committee on Employment of People with Disabilities is now accepting nominations for the 2008 employee, employer and services provider of the year. To make a nomination, please click the hyperlink below to open the forms. Once you can view the forms, you can ether save the forms locally to your PC (by left clicking the "save" button on your tool bar) or print them (by left clicking the "print" button on your tool bar). Once you have printed the forms, you can then fill in the required information. The instructions are also available for you to download and print. Deadline for submission of forms is JULY 31, 2008
