The forms below will be used by MA-64 to approve delegations of either Purchase Card or DOE/C-Web purchasing authority. Individuals should complete the form and get their supervisor to approve it. Then send the form to Ms. Deborah Black, MA-641 if applying for a purchase card or Ms. Beverly McCloskey, MA-644 if applying for DOE/C-Web authority. For DOE/C-Web authority, MA-644 will obtain the final two concurrences listed on the application.
