Lien Release on an Assignment of a Life Insurance Policy

Business Equipment, Business Property, etc. please provide us with the following required document(s):
  • A copy of the Collateral Assignment of Life Insurance Policy for the policy that you are requesting to be released. The Life Insurance Company that issued the policy should be able to provide this. The copy must be readable and clearly show the Name of the Insured Person, Date of Insurance Policy, the Policy Number, Name of the Insurance Company that issued the policy, and the address of the Insurance Company.

    Please remember that FDIC can only accept a release request when the lienholder is a Bank or Savings and Loan Institution that failed and has been placed in FDIC receivership, OR, a Subsidiary of a Failed Bank or Savings and Loan. It is highly suggested you also provide the following documents, if available:

  • Proof that the loan was paid in full. Proof of Payoff may include such things as a copy of a "PAID" Note, copies of payment checks, or anything else that would indicate payment. This will greatly reduce the amount of research time and expedite our handling of your request.

Where to send your request

  • A request for a Release of Lien must be made in writing.
  • You can mail your request to: FDIC, 1601 Bryan Street, Dallas TX 75201 Attention: Customer Service
  • OR it can be faxed to 703-812-1082
  • We ask that you either fax your request OR mail it in. Please do not do both. Hard to read documents should be mailed or sent overnight, rather than faxed. Unreadable documents cannot be processed.

Be sure to include your address and phone number with your written request.

Because of the large volume of release requests we get, it may take up to 30 business days
to obtain a release of lien so you may want to plan accordingly.
