OBJECTIVIES: To determine the epidemiological, clinical and therapeutic pecularities of severe strongyloidiasis in HIV infected patients. METHODS: Retrospective study of subjects presenting with severe strongyloidiasis in a cohort of 350 Western Blott confimerd HIV positive patients followed at CISIH (HIV information and care center) of Pointe a Pitre Guadeloupe between January 1991 and december 1993. RESULTS: 2 patients received corticosteroids a few weeks before the strongyloid infection; 1 patient had infection through probably homosexual intercourse. The patients are men aged 36 to 61 years. 4 were guadeloupean and the last one was living in Guadeloup for the past 20 years. The predominant clinical manifestations are pulmonary: n = 3 (strongyloides larva in B.A.L.), neuromuscular and renal: n = 1 related to severe hypokalemia and major GI: n = 1. The 5 patients have CD4 level below 100 and 4 below 50; in WHO stages of AIDS they belong to B3: n = 4 and C3: n = 1. Eosinophilia was greater 500/mm3 in 4, firstly n = 2 of after treatment: n = 2. In 2 cases there is anemia of acute onset: hb < 5 g/dl. Treatment includs variably: Albendazole, Tiabendazole or Ivermectine on a long term basis: n = 5 and repeatedly: n = 4. The clinical outcome was favorable in all the subjects with pulmonary sequelae: n = 1 or GI: n = 1. CONCLUSION: While not included in AIDS definition, severe strongyloidiasis should be considered in HIV + patients in endemic areas with advanced I. depression.
