Quantitation of cytomegalovirus (CMV) in cerebrospinal fluid (CSF) in AIDS patients by polymerase chain reaction (PCR).

Objective: To evaluate the performance of a prototype quantitative PCR assay for the detection of CMV infection in AIDS patients with neurological disorders. Methods: CSF samples collected from AIDS patients were tested for CMV by a prototype quantitative PCR assay (Roche Molecular Systems), the antigenemia (Ag) assay and the conventional tube culture. Results: A total of forty-four CSF samples from thirty-four AIDS patients were tested. Twenty-six out of the forty-four samples were from patients with polyradiculopathy (Pri). The PCR assay identified thirty CMV positive samples: twenty-four samples from Pri patients (24/26=0.92) and six samples from patients with non-Pri neurological disorders. With one exception, the CMV load in Pri patients measured by PCR ranged from 1x10(3) to 7.5x10(5) copies per ml CSF. The exception was a sample that had low CMV load (less than 100) and was from a patient undergoing ganciclovir treatment; another CSF sample taken from this patient seven months earlier had a CMV load of 2.7x10(5). The Ag assay was positive for twelve out of sixteen samples with Pri (12/16=0.75). Among the non-Pri patients, the PCR assay (50 microliters CSF per assay) was negative for six samples that were CMV positive by the Ag assay (1 ml CSF per assay). CMV load in non-Pri patients determined by PCR was usually less than 100 copies per ml CSF. The impact of treatment on CMV load as measured by PCR and the Ag assay is being evaluated. Conclusions: The sensitivity of PCR to identify polyradiculopathy was 92%. The sensitivity of the Ag assay and culture were 75% and 57%, respectively.
