Quinolone-Resistant Pneumococci from Croatia.

BACKGROUND: Preliminary data indicate a raised prevalence of pneumococci with high quinolone (QNL) MICs in Zagreb. METHODS: We collected 231 sequentially clinically isolated pneumococci from adults (8 hospitals, 6 Croatian cities, Nov 2000-Apr 2001) and tested them for susceptibility to a range of antibacterials by agar dilution in air. RESULTS: [table: see text] MIC[50]/[90]s (microg/ml) were: Of 231 strains, 65% were PEN[S], 19% PEN[I], 16% PEN[R]. Macrolide resistance (ERY MICs 0.5->64 microg/ml; 15% of strains) was seen only in PEN[R] strains (92%). 7 strains (3%) had LEV MICs >/=4.0 microg/ml: all were PEN[R] (MIC >2 microg/ml) but ERY[S]. 1 hospital, Zagreb, had 5 QNL[R] strains (3 from bronchial aspirates, 1 pleural fluid, 1 blood); Cakovec, 1 (sputum); Varazdin, 1 (blood). MICs (microg/ml) for the 7 strains = GEM, 0.125-0.5; GAT, 2-16; MOX, 2-4; CIP, >/=4 microg/ml, except for 1 strain (2 microg/ml). All 7 strains were type 23F, with mutations in ParC (S79F), GyrA (S81Y) and ParE (I460V). Apart from the Cakovec strain, the strains had the same pulse field patterns. CONCLUSIONS: The QNL[R] rate of 3% is similar to that reported in Canada. All QNL[R] strains were also PEN[R]. GEM gave the lowest MICs of all QNLs tested. Spread of these QNL[R] clones will complicate therapy.
