How to Report a Pet Food Complaint
To report complaints about pet food (and other animal feed), please contact you state FDA's Consumer Complaint Coordinator(s). Contact information can be found at the list of FDA's Consumer Complaint Coordinators. When reporting, please include as much information as possible, including the specific product name, lot numbers, veterinarian's report and diagnosis, etc.
Please use the subject line to indicate what your message is regarding. Use a subject line like "Structural elements" for problems such as non-working links, broken graphics, or special characters that are not loading properly. Use a subject line like "Content" for comments regarding the content of this site. Content issues would include specific questions about Freedom of Information Summaries, CVM Updates, or Guidelines. Please note: FDA cannot comment about drugs that are in the review process, e.g., we cannot answer a question about when a drug will be approved or not approved.

Veterinary Drug Adverse Reaction Reporting
If you are aware of adverse reactions to veterinary drugs, please report them to the FDA. Information about adverse drug reactions is critical to the CVM's on-going surveillance of animal drug products to assure their continued safety and effectiveness. You may report your experiences on a FORM FDA 1932a, "Veterinary Adverse Experience, Lack of Effectiveness or Product Defect Report". The 1932a is pre-addressed, prepaid postage form that is completed and dropped in the mail. A form may be obtained by writing:

You may also report adverse reactions by telephoning the Center for Veterinary Medicine at 1-888-FDA-VETS.
IMPORTANT: The identities all persons and animals is held in strict confidence by FDA and protected to the fullest extent of the law. The reporter's identity may be shared with the manufacturer or distributor unless requested otherwise. However, FDA will not disclose the reporter's identity to a request from the public, pursuant to the Freedom of Information Act.
