Consolidated Edison of Company of New York and Vermont Yankee
respectively with regard to the nuclear qualified pressure transmitters.
We will keep you informed of on-going developments and the completion of
seismic testing for the design.

Dear Mr. Bauer,
Per our conversation today, this letter is to inform you that an error
was made in the processing of your order for five (5) nuclear qualified
pressure transmitters. Foxboro inadvertently shipped the wrong version
of the electronics module, which at this time is being scheduled for
nuclear qualification testing but which has not undergone the nuclear
qualification testing. The modules shipped are in the same family as
modules already nuclear qualified and are therefore believed to also be
capable of withstanding the seismic requirements of nuclear
qualification. This opinion is supported by vibration test data, which
shows the design to be sufficiently robust. While this data is not
conclusive, we believe it is a strong indication of design capability.
On the basis of a review of the design and the vibration test data,
Foxboro does not consider this to be a safety issue, which would
constitute a substantial hazard.
Never-the-less, we are informing you so that you can evaluate the
applications you have for these units. Meanwhile, as stated, we are
taking immediate actions to qualify this design, within the next two to
four weeks, to the nuclear seismic requirements, and are taking
corrective actions to deal with the cause for the improper shipment. We
will also be informing the Nuclear Regulatory Commission of this
notification to you.

Subject: Your P.O. order #617927, serial #'s 97030077 for nuclear
qualified pressure transmitters shipped 3/11/97.
Dear Mr. Simone,
This letter confirms Foxboro's notification, through Alan Budnick,
General Manager-Quality Assurance Oversight on October 9, 1997, and
subsequent follow-up conversations with him and Steve Misilo regarding
the subject transmitters. Our records show that your order specified
6000 rads as a radiation requirement, though our standard part number was
called for and no certification of radiation was requested. The product
we shipped does not quite meet the 6000 rads total dosage. We have
supplied to your Mr. Steve Misilo the radiation test report which was
conducted so that he could evaluate the application and determine the
appropriate action. This test report indicated a capability of 4700 rads
total integrated dose (tid). We understand that this transmitter is used
in the reactor coolant system (RCS) but is valved out during reactor
operation, has no direct safety function, and is used only for cold shut-
down as a level measurement. We understand that this condition would not
affect plant operability but should be evaluated to determine if
transmitter life expectations are met. Foxboro awaits this determination
and will work out appropriate action with you.
