SUBJECT:     Request for Comments - Program Monitoring Report and Job Service Complaint Form, ETA Forms 5148 and 8429; OMB Number 1205-0039

PROGRAM(S):         Wagner-Peyser

1.  Purpose.  To transmit a request for public comment on an extension of an existing Information Collection Request (ICR), which has been submitted to the Office of Management and Budget for review and approval, concerning job service program monitoring and complaint reporting on ETA Forms 5148 and 8429, under the Wagner-Peyser Act.
