Freedom of Information Act Requests

All FOIA requests for the U. S. Office of Personnel Management (OPM) must be sent to the mailing address or, alternatively, to the email address shown below:

Privacy Act Requests

OPM also receives a large number of Privacy Act requests for background investigations and former Federal employee personnel records. For your convenience, the instructions for obtaining these types of Privacy Act requests are below. If you prefer, you may make your request directly to the FOIA Requester Service Center noted above; we will forward your request to the office below. You may view our privacy policy at http://www.opm.gov/html/privacy.asp

Background Investigations

To obtain a copy of your background investigation conducted by OPM's Federal Investigative Services Division, please mail your request to:

Your request must include your hand written signature and all of the following information:

  • Full name
  • Social Security Number
  • Date of birth
  • Place of birth
  • Current home address (a Post Office Box is not acceptable; the records are sent by certified mail and require your signature).

Alternatively, you may FAX your hand signed request to telephone number 724-794-4590.

Personnel Records for Current Federal Employees

The Official Personnel Folders (OPF), including SF 50's, for current Federal employees are stored at the employee's current employing agency. The request for the records must be made directly to that agency's Freedom of Information/Privacy office. The Department of Justice (DOJ) lists the current agency Freedom of Information/Privacy Contacts on the DOJ website.

Personnel Records for Former Federal Employees

The Official Personnel Folders (OPF), including SF 50's, for most former Federal employees are stored at the address below. Please mail your request to:

National Personnel Records Center (NPRC)

Alternatively, you may FAX your hand signed request to NPRC at telephone number 314-801-9269. More information is available on the NPRC website.

The Official Personnel Folders (OPF) for former Postal employees are stored at the address below. Please mail your request to:

Your request to either NPRC or the U.S. Postal Service must include your hand written signature and all of the following information:

  • Social Security Number
