eOPF Master Forms List

This interactive database includes four forms lists as part of the eOPF Master Forms List. The eOPF follows the Guide to Personnel Recordkeeping and the Guide to Processing Personnel Actions to determine the eOPF forms. In addition, as agencies convert to eOPF, they have identified agency specific forms. The Master Forms List is not all inclusive; the forms listed therein have been submitted by agencies migrating to eOPF. Each agency has discretion to define additional forms to address the agency’s specific needs.
