Outreach services target older people in Illinois communities who are difficult to contact and who may not be aware that senior services are available in their communities.

Outreach staff conduct home visits with older persons (and/or their family members and other caregivers) to inform them about available public benefits and services, encourage older persons to participate in local senior programs and link them to needed assistance.
Specially trained outreach workers can share facts about available services and public benefits programs to help older persons and those who care for them. An older person or a family caregiver can receive up-to-date information on a wide range of community-based programs such as respite services, transportation, home-delivered meals, homemaker services, etc.
Outreach workers can also provide information on prescription drug assistance programs, income tax assistance programs, property tax assistance programs, Medicare, Medicaid and many more.
