The Centers for Medicare & Medicaid Services (CMS) Standardized Provider Inquiry Chart provides common inquiry categories and definitions for contractors to use when tracking and reporting the nature of your inquiry types for telephone and written provider inquiries. These categories are to be used to capture the reason for the inquiry. Contractors are required to submit a Contractor Inquiry Tracking Report for telephone and written inquiries every quarter. Contractors must complete entirely the 2 worksheets contained in the report.
