Comments:Outcomes include mortality rates, major complication rates, and "failure to rescue" rates (deaths that occur in the hospital after a major complication). Report includes volume measures grouped into four clinical categories; cardiac care, general surgery, orthopedic surgery, and pulmonary disease. The printed version of the report includes process measures for heart failure care and pneumonia care.

Comments:For each of 10 conditions/procedures, hospitals receive from 1 to 5 stars. The number of stars represents a combined score for the four measures as compared to the average performance of all hospitals in the State. Hospitals are then grouped by the number of stars they received, e.g., all hospitals that received five stars for cardiac catheterization are grouped together. Symbols are used to denote hospital's performance for the two process measures.
