What Methyl Bromide Reporting Forms Should I Complete?

There are 11 Methyl Bromide reporting forms available for use from EPA:

  1. The Methyl Bromide Producer Quarterly Report should be completed by any entity that is producing methyl bromide. For more information about completing the Producer Report, please refer to Hints for Completing the Methyl Bromide Producer Quarterly Report.

  2. The Methyl Bromide Importer Quarterly Report should be completed by any entity that is importing methyl bromide from outside of the United States or U.S. Territories. Please note that any shipments of methyl bromide deriving from U.S. Territories (i.e., Puerto Rico, U.S. Virgin Islands, American Samoa, Guam, or Northern Mariana Islands) are not considered imports.   3. The Methyl Bromide Exporter Quarterly Report should be completed by any entity that is exporting methyl bromide outside of the United States or U.S. Territories. Please note that any shipments of methyl bromide to U.S. Territories (i.e., Puerto Rico, U.S. Virgin Islands, American Samoa, Guam, or Northern Mariana Islands) are not considered imports.   4. The Notification of Methyl Bromide Transfers should be completed by any entity that is transferring their Pre-plant or Post-harvest Critical Use Allowances, Article 5 Allowances, or their Critical Stock Allowances, to another entity.   5. The Distributor of QPS Methyl Bromide Quarterly Report should be completed by entities that distribute Methyl Bromide for Quarantine and Pre-shipment application purposes.   6. The Certification of Order/Purchase of QPS Methyl Bromide Report can be used by distributors or 3rd party applicators purchasing methyl bromide for QPS application purposes. This form certifies to the seller of QPS methyl bromide that the buyer will use the methyl bromide solely for QPS application and will not resell or use the QPS methyl bromide in manufacturing. This report is NOT collected by EPA. It is completed by purchasers of QPS Methyl Bromide and provided to sellers of QPS Methyl Bromide.   7. The Certification of Order/Purchase of Critical Use Methyl Bromide Report can be used by distributors or 3rd party applicators purchasing critical use methyl bromide. This form certifies to the seller of critical use methyl bromide that the buyer is an approved critical user and will use the methyl bromide for an approved critical use. This report is NOT collected by EPA. It is completed by purchasers of Critical Use Methyl Bromide and provided to sellers of Critical Use Methyl Bromide.   8. The Sales of Critical Use Methyl Bromide to End Users Report should be completed by any company that provides Methyl Bromide gas or fumigations to critical users. This form should be completed by the entity’s central office on behalf of their divisions; ONE form per company.   9. The Pre-2005 Methyl Bromide Stocks Report should be completed by any company that (A) received a critical stock allowance, (B) bought a critical use allowance, or (C) owns stocks of methyl bromide that were produced or imported prior to January 1, 2005.
