Pesticide Production Report & Establishment Registration

The annual Pesticide Report for Pesticide-Producing Establishments (EPA Form 3540-16) must be submitted on or before March 1 of the calendar year. Every establishment registered with EPA as a pesticide-producing establishment must submit this report.

A facility with an active EPA establishment number must submit a report even if there was no production, or be liable for a fine of up to $5,500. Reporting requirements

Q. If you need an EPA Establishment Number :

A. Contact the Pesticide Section of the EPA Regional Office which has jurisdiction over your company headquarters' site location for an application. If the main office of your company is located in Connecticut, Maine, Massachusetts, New Hampshire, Rhode Island or Vermont, Region 1 is responsible for assigning establishment numbers regardless of where your other facilities may be located, and you should contact:

Q. If you no longer want or need an establishment number

A. Contact Kan Tham as previously identified to inactivate your establishment number. The number will not be deleted and can be reactivated at any future time at your request.

However, if you inactivate or cancel your establishment, pesticide production must not occur in your facility. Any pesticide production in an unregistered establishment or an establishment whose registration has been inactivated is an unlawful act under FIFRA and may be subject to civil and/or criminal actions.
