Acute abdominal pain is a common complaint of patients coming to a primary care physician. A primary objective of the initial patient evaluation is to determine if the presentation requires emergency evaluation and therapy. This chapter on acute abdominal pain is from a book that focuses on the most common gastroenterological problems encountered in a primary practice setting. The chapter is organized to support rapid access to the information necessary to evaluate and treat most patients with this problems. Topics include the prevalence of acute abdominal pain; principal diagnoses, including gastric ulcer, duodenal ulcer, acute cholecystitis, acute pancreatitis, acute small bowel obstruction, acute mesenteric ischemia (lack of blood flow), acute appendicitis, large bowel obstruction, and acute diverticulitis; the typical presentation of each of these conditions; the recommended physical examination and ancillary tests including complete blood cell count, blood chemistry, abdominal x ray, barium radiography, ultrasonography, hepatobiliary scanning, computer tomography (CT) scan, angiography, and endoscopy; treatment options for each of the diagnoses; and clinical errors.
