Subject:  FORMS - Well schedule revision

During the past several years there has been considerable
discussion regarding the desirability of revising the Well
Schedule Form (9-185), and two memoranda have been sent to the
Field offices asking for opinions on the subject.

The replies from the Field offices have been studied carefully,
and a revised experimental well schedule prepared that
incorporates as many as possible of the suggested revisions.  It
is recognized that this schedule will not meet the needs of all
the Field offices, and that some of the offices may need to
continue using specially designed forms, or supplemental forms
that meet special requirements in particular areas.  Some special
requirements may be met by modifying the well schedule by rubber
stamping on the reverse side.

The Field office voted overwhelmingly in favor of retaining the
present 5 X 8 inch form with the back side blank.  With these
restrictions, it was possible to make only a few of the many
changes and additions requested by the Field offices.  The
recommended changes and additions were tabulated and the ones most
frequently recommended were made.  Space for the additional
requested items was made by better utilizing the space at the top
of the schedule and by reducing the space allowed for some of the
items.

Spaces for the well number have been incorporated at both the top
and bottom of the sheet.  The well number space at the bottom was
provided to make it easier to index the schedules when a bunch of
schedules are kept on a clip board or in a book.  The well number
space at the bottom was placed on the lower right-hand side to
make it easier to index the schedules when filed in a standard 5 X
8 inch file.  Item 6 was arranged to provide space for casing
diameters where more than one size of casing is used in a well.
Item 8 was enlarged to allow space for both reported and measured
depths.  Frequently the reported depth to water at some previous
date is significant.  Item 9 was designed to provide space for
additional data on pump performance.  Item 12 provides space for
recording chemical quality date, but eliminates some of the wasted
space that had been used to record data only infrequently.  The
space thus saved was added to the space for "Remarks" which can be
used to record additional chemical quality data if needed, but
also may be used to record other miscellaneous data.

Enclosed are copies of the revised experimental well schedule.
Each Field office is requested to study the schedule carefully
and fill out the attached form and return it to this office by
April 15, 1958.

A few of the Field offices have expressed an interest in the use
of Keysort system for recording well data.  This system has its
advantages and disadvantages and because of the large amount of
time and effort that would be required to change to and maintain
this system, it is felt that the subject should be thoroughly
studied.  In order to help this office determine whether there is
sufficient interest in and need for this system to justify a
study, please answer the questions regarding the Keysort system.
