Important Info About Fraudulent Email, Phone, or Text Messages and Reliacard
ReliaCard
Many of you receive unemployment insurance benefits on Reliacard. Any email or text message that asks you to confirm, verify, or otherwise provide personal information (also known as phishing) regarding your Reliacard account is a fraud.

US BANK will never ask you for sensitive account information such as your Passwords, PIN Numbers, Social Security Number or Account Numbers via email, phone or text message. The Employment Department will never ask you for your Passwords, PIN Numbers or Bank Account Number information via email, phone or text message.

If you have any questions about any message received from US Bank, call the phone number on the back of your ReliaCard Visa Debit card (1-866-276-5114). If you have any questions about any message received from the Oregon Employment Department, call your UI Center.

Benefit Extensions Currently Available
EUC Tier 1 provides up to 20 weeks of additional benefits to claimants exhausting regular benefits, or whose regular claim expired after May 1, 2007. The last week to apply for EUC Tier 1 benefits is the week of December 20-26, 2009.

EUC Tier 2 provides up to 13 weeks of additional benefits to claimants who exhaust Tier 1 benefits with the payment of the week December 13-19, 2009, or before. You do not need to apply for Tier 2 benefits.

EB provides an additional 13* weeks of additional benefits to claims that expire after December 7, 2008.

*High EB expands EB from 13 to 20 weeks of benefits starting February 15, 2009.

To receive any extenstion, you cannot qualify for regular unemployment insurance benefits in Oregon or any other state.
American Recovery and Reinvestment Act of 2009

Unemployment Insurance and Taxes - Form 1099-G
Form 1099-G for 2008 is now available in the Online Claims System, under Status of your claim. To access the information, you will need to enter your Social Security number and your PIN number. Your PIN number is the four digit number used to claim your weekly benefits.

All 1099-G forms for tax year 2008 have now been mailed. If you do not receive it, and you do not remember your PIN number to get the information online, contact your UI Center to update your address and request a duplicate copy be mailed.
