Combined Assessment Program Review of the VA San Diego Healthcare System San Diego, California

The purpose of the review was to evaluate selected operations, focusing on patient care administration and quality management (QM). During the review, we provided fraud and integrity awareness training to 328 employees. The review covered eight operational activities and a follow-up review area from the prior CAP review. The system complied with selected standards in the following three areas: (1) coordination of care, (2) nurse staffing, and (3) patient satisfaction survey scores. We highlighted positive achievements in developing a computerized patient event reporting system, executing emergency response activities in reaction to wildfires, and creating a user-friendly ordering and documentation system for patients with acute coronary syndrome. We made recommendations for improvements in QM, pharmacy operations, environment of care, emergency/urgent care operations, medication management, and breast cancer management.
