SUBJECT:   Missing Pages for the 1996 Annual Agency
           Ethics Program Questionnaire

     Pages 13 and 14 may have been inadvertently omitted from your
copy of the 1996 Annual Agency Program Questionnaire distributed 
on October 23, 1996.  Please include the attached pages 13 and 14 
when completing the 1996 questionnaire.

     In addition, the second page of the listing of agencies
reporting in 1995 may have also been omitted.  Please find a copy
of this page attached to this memorandum.

     The Office of Government Ethics apologizes for any 
inconvenience this may cause.
