REGISTRATION PROCESS:
Step 1: Download/Complete Nomination Application Form & Append Nomination Narrative--->
Step 2: Upload Completed Nomination Document --->
Step 3: Review ---> Step 4: Receipt --->

Instructions: This is the first of four steps.
Step 1: Complete your Nomination Document by following the steps below.
  1. Download the Nomination Application Form to your local drive/desktop.
    (Click "Save" if asked to Open or Save file, or right-click mouse on file then "Save Target As")
    • CTC_2009_application.doc
    • CTC_2009_application.rtf

  2. Open the Nomination Application Form and click the Enable Macros button if necessary. Complete the form.
    (Macro security settings can be managed -- type "enable macros" in your Word search box to find out how your version of Word works)


  3. Append your Nomination Narrative to the end of the Application Form.
    (Remember: Narrative is limited to 4 pages and 1.5MB in size including any embedded images.)


  4. Indicate whether you CONCUR or DO NOT CONCUR with your facility's/installation's information contained within EPA's OTIS database.


  5. Save the file with a unique name.


  6. Return to this page and click the continue button below.
