Our services and supports help eligible individuals with developmental disabilities achieve self-sufficiency and independence. The Division likewise offers supports for family members and other caregivers. We believe that you can best be supported in integrated community settings and the majority of the Division’s programs and services are tailored to meet individual needs at home and other community settings.

PROVIDER NEWS
The Uniform Billing template (26 KB XLS) uses new document specifications (153 KB PDF), including the required Master Carrier ID (334 KB PDF). The rounding amendment to the template helps providers insert formulas to calculate their totals properly. Get complete instructions in the Provider Billing Reference Guide (1.08 MB PDF). When a provider bills an insurance company and the insurance company denies the service, the Waiver Request Form (21 KB XLS) must be completed and sent to us as documentation of the denial. Upon receipt of your form, we can enter a waiver into FOCUS, making the Division the primary payor for the calendar year.

POLICY NEWS
The Division Developmental Disabilities has revised Chapters 100 (41 KB PDF) “Introduction” and 500 (234 KB PDF) “Eligibility” of its Policy and Procedures Manual. In addition to updated terms and language, each policy contains a revised “Table of Contents” at the beginning of the chapter. An “Index” has been provided at the end of the chapter for easier navigation within the policy manual. Electronic links to policy sections, laws, rules and booklets have also been added. The revisions to Chapter 100 provide further details regarding policy manual organization, the policy development/review process and policy distribution.
Changes to Chapter 500 include further information regarding the application process and eligibility requirements for the Division, residency and the Arizona Early Intervention Program (AzEIP). You will also notice the term “cognitive disability” has been included in Chapter 500.
