Official Form - Exhibit D Print

Beginning February 2, 2009, the Eastern District of Wisconsin Bankruptcy Court's CM/ECF system has been updated to include additional events related to the credit counseling requirement.  We have notified Best Case, EZ Filing, and New Hope of this change. These new events should be utilized dependent upon the box checked on Official Form - Exhibit D.  Listed below are the four boxes on Exhibit D and the corresponding CM/ECF event.

Privacy Rule Reminder Print
Privacy Rule Reminder: To protect the rights of debtors and creditors, Bankruptcy Rule 9037 requires that certain personal data identifiers be modified or redacted from Bankruptcy Court case files. While there are limited exceptions, only the following should appear in filings made with the Bankruptcy Court:

-           only the last four digits of an individual's social security number or taxpayer identification number
-           only the last four digits of a financial account number
-           only the year of an individual's birth
-           only the initials of a minor.

Please note it is the responsibility of the attorney or the party making the filing with the court to redact the personal information. The clerk will not review documents and remove personal information. Special rules apply for transcripts that are filed with the Court. See the Transcripts tab of this website for more information. Consult Bankruptcy Rule 9037 for exemptions from the redaction requirements. If you or another party in a case has filed personal identifiers, you may file a Motion to Restrict Viewing of that document to court staff only. Please contact us with questions about the Privacy Rule.

Pro Se Help Desk Print

Pro Se Help Desk, Thursdays, 9:00 a.m. to 10:30 a.m., U.S. Courthouse, Room 133B.

We need volunteer attorneys for the Pro Se Help Desk and attorneys who may be willing to accept referrals of Chapter 13 debtors. If you are an attorney, and you wish to volunteer or add your name to the referral list, please contact the Clerk's Office at (414) 297-3291, extension 3204.

The bankruptcy section of the Milwaukee Bar Association, with the assistance and support of the State Bar of Wisconsin, provides a Pro Se Bankruptcy Help Desk at the United States Bankruptcy Court in Milwaukee. The service is provided on a first come, first served basis.  Due to increased volume, it is possible that if you do not come early you may not be seen. There will be a sign-in form outside room 133B and each person will be called in turn.

The service is available to chapter 7 debtors (or people considering filing chapter 7 bankruptcy) who wish to proceed pro se (i.e., do not have a lawyer to represent them in a bankruptcy case).

The service provides assistance with completing the required bankruptcy paperwork.
