Program Description
The Special Diabetes Program for Indians Healthy Heart Demonstration Project at the Yakama Indian Health Service is located in central Washington State. Project staff include a project coordinator, data coordinator/project assistant, lead case manager, and project director/program physician.
Successful Activities
One of the program’s most successful activities is its weekly "Dance Away Diabetes" class. Rocco Clark Sr., a former competitive dancer, teaches the dances along with powwow rules, etiquette, and tradition. The first Yakama Healthy Heart Dance Away Diabetes Powwow occurred in May 2007. The Yakama Healthy Heart Community Track Meet is another successful activity sponsored by the program. During these activities, the program offers diabetes education and healthy snacks.
Elements of Success
- Program staff are familiar with the community and had established trust with and respect for the community prior to the project's inception. This has made their recruitment efforts very successful. In fact, family members of participants frequently express interest in joining the Healthy Heart Demonstration Project.
- A pharmacist who serves as a case manager coordinates the filling of participants’ prescriptions. This is a unique way to accomplish the target goals of the Healthy Heart Demonstration Project.
- The program uses community members in advertising posters and develops many of their own promotional and educational materials, making them specific and relevant to the Yakama Tribe.
