Procedure for Current Contractors to Obtain HSPD-12 Card

I.   Verify or Initiate Personnel Investigation

All current GSA contractors who require routine access to GSA facilities or IT systems must have a suitable personnel investigation completed or in process by October 27, 2008, in accordance with HSPD-12 guidelines.  To find out whether you have a personnel investigation completed or in process, contact your Contracting Officer (CO) or Contracting Officer’s Technical Representative (COTR). 

If you need to initiate a personnel investigation, complete the following steps:

  1. Contact your Contracting Officer (CO) or Contracting Officer’s Technical Representative (COTR) to complete the Contractor Information Worksheet (CIW) and to have the CO or COTR set up an investigation request in the Office of Personnel Management (OPM) e-QIP system.  You will then receive an e-QIP invitation by email. 
  2. Log on to OPM’s e-QIP system and complete the online investigation form per instructions in the email invitation.
  3. Print and sign all required signature forms after entering all the necessary information in e-QIP.
  4. Make a copy of your signature forms for your own records and forward the signed forms to your CO or COTR.

 

II.   Enroll, Pick Up, and Activate HSPD-12 Card

The HSPD-12 Program Management Office (PMO) is currently working to sponsor contractors who require a HSPD-12 card.  Those contractors should have received the sponsorship notification email from the HSPD-12 PMO to inform them that they have been sponsored and to expect to receive an email from the GSA Managed Service Office (MSO) (HSPD12Admin@identitymsp.com) with instructions on how to schedule their enrollment appointment.  If you haven’t received an email from the HSPD-12 PMO that you have been sponsored, please contact your HSPD-12 Point of Contact.

Enrollment

Contractor receives an email from the GSA HSPD-12 Program Management Office (PMO) (HSPD12PMO@gsa.gov) that they have been sponsored for their HSPD-12 card and that they will receive an email from the GSA Managed Service Office (MSO) (HSPD12Admin@identitymsp.com) with instructions on how to schedule their enrollment appointment.  The PMO’s email will also ask the contractor to contact the GSA HSPD-12 PMO if they do not receive the MSO email within 48 hours.  (Prior to sponsorship, the HSPD-12 PMO receives the sponsorship request from the HSPD-12 Points of Contact.)  A favorable personnel investigation adjudication result or at least a favorable initial suitability check decision (fingerprint check) is required before sponsorship is complete.  

After receiving the instructions on how to schedule an enrollment, the contractor clicks on the link and follows the instructions in the Managed Service Office’s (MSO) email to create a user account and schedule an enrollment appointment at an available enrollment center.  (Contractor should save their user account information because they will need to use it to schedule their card activation appointment later.)  The registrar asks the contractor to provide two forms of approved ID and takes the contractor’s photograph and scans their fingers for fingerprints.

Card Pick Up and Activation

Contractor receives an email from the HSPD-12 Program Management Office (PMO) (HSPD12PMO@gsa.gov) to inform them that they will receive an email from the Managed Service Office (MSO) (HSPD12Admin@identitymsp.com) with their one-time activation PIN, card pick up information, and link to MSO online scheduling system to schedule card activation. 

The PMO’s email will instruct the contractor to disregard the card pick up location information because it is incorrect but retain the PIN and use the link to schedule their card activation appointment.  The contractor will receive the correct card pick up information in an email from their local Regional Credentialing Officer. 

The contractor schedules their card activation appointment by clicking on the link in the MSO card activation email notification and by using the same user account information they used to schedule their enrollment appointment. 

Contractor receives an email notification from their Regional Credentialing Officer (RCO) to inform them that their card is available to be picked up and will specify the card pick up location. 

Contractor then picks up their card from the RCO at the location specified in the RCO’s email.  Contractor provides proof of ID to the RCO to receive their card and will turn in their Non-HSPD-12 or temporary badge or pass.  (Contractor may wish to pick up their card shortly before their activation appointment if the location they selected is near where they will pick up their card.)

Lastly, contractor provides their proof of ID to the registrar and brings their one-time activation PIN to activate their card. 

Note: Credentialing procedures will change as a result of deployment of GSA credentialing IT system and continuous process improvements.  This procedural guide and other HSPD-12 related information will be updated as need to reflect changes.

III.   Available Help

Procedure for New Contractors to Obtain HSPD-12 Card

Step

Description

1.      

Provide information for Contractor Information Worksheet (CIW) obtained from GSA Requesting Official.

 

The contractor provides the information to complete sections 1-4 of the Contractor Information Worksheet (CIW).  The GSA Requesting Official completes section 5 and may assist the contractor with completing all or part of sections 1-4.  (Federal Protective Service completes section 5.)  The GSA Requesting Official may be the Contracting Officer, Contracting Officer’s Representative, Contracting Officer’s Technical Representative, Project Manager, PBS Building Manager, or local HSPD-12 Point of Contact (POC).

Contractors without an e-mail address of their own should use their contractor company’s e-mail address. 

 

2.      

Provide fingerprints to a fingerprint service provider selected by the GSA Requesting Official.  Bring two forms of identity source documents in original form.

 

The Requesting Official schedules an appointment for the contractor to have their fingerprints taken by a fingerprint service provider.  The fingerprint service provider may be a local police station, Federal Protective Service (FPS), or GSA live-scan station, GSA HR department, or other agency fingerprinting facility to submit fingerprints.

The contractor’s two forms of ID must come from the list of acceptable documents included in Form I-9 OMB No. 1115-0136, “Employment Eligibility Verification.”  At least one document shall be a valid State or Federal Government-issued picture ID.

If paper fingerprint cards are used, the fingerprint service provider must use FD258 Fingerprint Card, and the contractor may not return their own fingerprint card to the Requesting Official.  Requesting Official should provide contractor with preaddressed, sealable envelop that the fingerprint service provider can use to return the fingerprint card by mail, overnight express, or interoffice mail. 

 

3.      

Complete personnel investigation forms in e-QIP system after receiving email invitation from Federal Protective Service (FPS).

 

The contractor logs on to the Office of Personnel Management (OPM’s) Electronic Questionnaires for Investigations Processing (e-QIP) system and completes the online investigation form per instructions in the email invitation from Federal Protective Service (FPS).  e-QIP allows applicants to electronically enter, update, and transmit their personal investigative data over a secure Internet connection to their employing agency for review and approval. Contractors with Prior Investigations

In accordance with HSPD-12 guidelines, GSA will not readjudicate contractors with previous personnel investigations, provided the following requirements are met:

·         FPS can verify with OPM and/or the agency that performed the adjudication the following conditions:

o        The prior investigation was completed.

o        The investigation results were favorable.

o        The granting agency has not received any significant derogatory information that was not previously adjudicated.

·         The contractor has had less than a 2-year break in service since the end of the previous contract.

·         The prior investigation is at the same or higher level as the personnel investigation that would be requested for the new contract.

In order to process a contractor with a previous personnel investigation, the Requesting Official needs to provide FPS with a CIW, indicating in Section 1 of the form that there was a prior investigation and the date of the investigation and indicating in Section 6 (Comments/Notes) the name of the agency that performed the adjudication.

FPS will notify the adjudicating agency and request verification of the successful adjudication.

If the previous agency adjudication information is not available or cannot be verified, the applicant should submit the personnel investigation forms to FPS who will forward them to OPM. OPM disseminates case files or information from previous investigations upon receipt of the completed personnel investigation questionnaire form (i.e., SF-85P).  The applicant's signature of consent will allow OPM to release the investigation file to the requesting agency.

 

4.      

Make a copy of all three signature forms of personnel investigation request and forward to GSA Requesting Official. 

 

Contractor makes a copy of signature forms for their own records and forwards signed copies to Requesting Official.  Requesting Official reviews the Contractor Information Worksheet (CIW) and forwards the CIW, fingerprint cards, and signature forms to the HSPD-12 Point of Contact (POC).  The HSPD-12 POC then sends the entire package to Federal Protective Service (FPS) to conduct the personnel investigation. 

 

5.      

Obtain Temporary Badge from your GSA Building Security Office.

 

Employees and contractors awaiting issuance of their HSPD-12 card can use the temporary badge in place of a HSPD-12 card when entering GSA-managed buildings after signing in and providing a suitable government photo ID. In addition, if an employee or long-term contractor forgets his/her card on a particular day, or if the person is waiting for a replacement HSPD-12 card, they can also be issued a temporary badge after their identity has been confirmed.

To obtain a temporary badge at GSA Central Office, the contractor must have received at least a favorable initial suitability decision and must complete the Personal Data Worksheet (PDW).  The Requesting Official escorts the contractor to the Building Security Office in B-232 where the Requesting Official provides the PDW, and the contractor presents a valid photo ID and signs for the temporary badge.  Requesting Officials in other GSA facilities should check with the local Regional Credentialing Officer for the procedures for contractors to obtain a temporary badge in their facility. 

 

6.      

Create Managed Service Office (MSO) Online Scheduling System user account and schedule enrollment appointment by following instructions and using link provided in email from MSO.

 

Contractor receives an email from the GSA HSPD-12 Program Management Office (PMO) (HSPD12PMO@gsa.gov) that they have been sponsored for their HSPD-12 card and that they will receive an email from the GSA Managed Service Office (MSO) (HSPD12Admin@identitymsp.com) with instructions on how to schedule their enrollment appointment.  The PMO’s email will also ask the contractor to contact the GSA HSPD-12 PMO if they don’t receive the MSO email within 48 hours.  (Prior to sponsorship, the HSPD-12 PMO receives the sponsorship request from the HSPD-12 Points of Contact (POC).  A favorable personnel investigation adjudication result or at least a favorable initial suitability check decision (fingerprint check) is required before sponsorship is complete.

Pick up your card from your Regional Credentialing Officer and turn in your non-HSPD-12 card, Temporary Badge, or Visitor Pass.

 

Contractor receives an email notification from their Regional Credentialing Officer (RCO) to inform them that their card is available to be picked up and will specify the card pick up location. 

Contractor picks up their card from the RCO at the location specified in the RCO’s email.  Contractor provides proof of ID to the RCO to receive their card and will turn in their Non-HSPD-12 or temporary badge or pass.  (Requesting Official may wish to advise contractor to pick up their card shortly before their activation appointment if the location they selected is near where they will pick up their card.)

 

10. 

Activate card at scheduled activation appointment.  Bring form of ID and one-time activation PIN.

 

Contractor provides their proof of ID to the registrar and brings their one-time activation PIN to activate their card.
