1993
GTE holding company acquired Contel of the South, Inc. which included Contel's
study area jurisdictions Alabama (COAL) and Georgia (COGA). Then, on 10/31/93,
GTE and ALLTEL traded study area jurisdictions. GTE gave ALLTEL study area COGA
and GTE received study areas Indiana (GLIN) and Michigan (GLMI).

Contel of the South, Inc. d/b/a GTE Systems of the South had no changes during
this period.
2000
On 6/30/2000, Bell Atlantic (BNTR) and GTE Corporation (GTTC) merged into one
company, Verizon Communications. Contel of the South, Inc. d/b/a GTE Systems of
the South was renamed Contel of the South, Inc. d/b/a Verizon Mid-States.
2001
Contel of the South, Inc. d/b/a Verizon Mid-States had no changes during this
period.
2002
Contel of the South d/b/a Mid-States sold study area jurisdiction Alabama (COAL)
to CenturyTel on 6/30/2002.

Contel of the South, Inc. d/b/a Verizon Mid-States fell below the revenue
threshold, when it sold study area - COAL. Therefore, the operating company and
