Medical Clearances

The NOAA Health Service Questionnaire (revised 4/04)(available here in Adobe Acrobat format) must be completed in advance by each participating scientist. It should reach the appropriate health services representative (see below) no later than 4 weeks prior to the cruise. This will allow time to medically clear an individual and to request more information if needed, as well as to help prepare for special circumstances. The forms will be sent out by the Chief Scientist. It is requested that all personnel bring any prescription medication needed and any over the counter medicine that is taken routinely (i.e., an aspirin a day). The ship does not stock medication for routine dispensing.
