Toolkit materials can be downloaded using the list of links on the right side of this screen > > >

Material descriptions are as follows:

Pertussis Tools:

• School Nurse Fact Sheet:
An educational fact sheet that provides you with information about the signs and symptoms of pertussis, how it is spread, rising pertussis rates, the vulnerability of adolescents, and the need for a Tdap (tetanus toxoid, reduced diphtheria toxoid, and acellular pertussis vaccine) booster shot.  This fact sheet can help educate students, parents, and the community about pertussis and what they can do to prevent it.

• Pertussis Slide Kit: A presentation designed to educate you and the school community (faculty, administration, PTA) about pertussis in adolescents and the need for a Tdap booster shot.  This presentation can be used to enlist support for pertussis education efforts.  NOTE: The Kit is available in two formats; Microsoft PowerPoint (PPT) and Adobe PDF.  If you want the PPT version, select "read only" if presented with a Password pop-up window.

• Whooping Cough Awareness Poster: This poster can be displayed in your office or in high-traffic areas throughout your school to raise awareness of the risks of pertussis and the need to vaccinate against whooping cough.

• Dear Parent Letter:
A sample letter that you can send to parents to educate them about the risks of pertussis in adolescents and the need for a Tdap (tetanus toxoid, reduced diphtheria toxoid, and acellular pertussis vaccine) booster shot for their children.

• What Parents Need to Know About Whooping Cough: A Q&A-style fact sheet, in both English and Spanish, with information about the signs and symptoms of pertussis, how it is spread, rising pertussis rates, the vulnerability of adolescents, and the need for a Tdap booster shot.  This educational fact sheet can be photocopied and distributed to parents and members of the community to help them learn about pertussis.

• Pertussis Outbreak Template Letter: A template you can use to send a letter to parents, schools administrators, local newspapers, or the community in the event of a pertussis outbreak.  The letter contains important information about what to do in case of an outbreak and how to prevent outbreaks from occurring in the future.

Hep A Tools:

• Hepatitis A School Nurse Fact Sheet:
An educational fact sheet that explains that hepatitis A often occurs in school environments.

• Hepatitis A Awareness Poster: This poster defines and details the signs and symptoms of hepatitis A and provides guidance for protecting students' health.  This poster can be displayed in your office or in high-traffic areas throughout your school.

• Dear Parent Letter: A sample letter that explains to parents the need for prevention and vaccination against hepatitis A, and may be particularly useful when hepatitis A outbreaks are reported in the media.
