Service Coordination is a relatively new field. It is imperative that
Service Coordinators educate others about their positions and responsibilities.
In addition, Service Coordinators must closely track their program's
performance, to make sure they are working as effectively as possible.
HUD's Semi-Annual
Performance Report (form HUD-92456) is designed to facilitate
these activities.
HUD
staff want to know the details of a Service Coordinator's work and
make sure the program is operating according to outstanding requirements.
Service Coordinators need to know how many people they are serving,
what kinds of services are obtained, how much time is spent on administrative
paperwork, etc. Service Coordinators can use their completed reports
to show the results of their work to such people as supervisors,
prospective tenants, and third-party funders.
All
Service Coordinators in HUD assisted housing, whose positions are
paid for by grant funds, Section 8 or PRAC operating funds, residual
receipts, or excess income, must submit this report. The two reporting
periods are October 1 through March 31 and April 1 to September
30. Reports are due to Field office representatives no later than
30 days following the end of each reporting period.
