Healthy Connections is a mandatory Primary Care Case Management program for Idaho Medicaid.  Most people participating in either Medicaid benefits plan (Basic or Enhanced) must enroll in Healthy Connections.  Healthy Connections enrollment means you choose one doctor or clinic who will manage your healthcare.
