NTSB Recommendation: NTSB recommends that the PHMSA an operator to revise its pipeline risk assessment plan whenever it has failed to consider one or more risk factors that can affect pipeline integrity.

PHMSA Action: A letter requesting closure on this recommendation was sent to NTSB in Fall 2008.
