The National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) consists of the Division of Adolescent and School Health, the Division of Adult and Community Health, the Division of Diabetes Translation, the Division of Nutrition and Physical Activity, the Division of Reproductive Health, and the Office on Smoking and Health. NCCDPHP plans, directs, and coordinates a national program for the prevention of premature mortality, morbidity, and disability due to chronic illnesses and conditions and promotes the overall health of the population. NCCDPHP maintains five databases -- Health Promotion and Education Database, Comprehensive School Health Database, Cancer Prevention and Control Database, Prenatal Smoking Cessation Database, and the Epilepsy Education and Prevention Activities Database.
