The Firework Program permits additional item name(s) to be added to a valid EX Approval number, currently held by a company. Any request for an additional item name will require the company to certify that the device is identical to the device described in the original application filed with the Office of Approvals. Each time a request is submitted and approved, the Office of Approvals will reissue the same EX Approval number, listing the original item name and the subsequent item names used for the device. EX Approvals that are reissued will be issued with a 5 year expiration date.
The procedures for submitting a request for additional item name(s) follows:
- Submit a signed and dated letter of Request which includes the EX Number of the approved device, the original item name and all additional names associated with the EX Number,
- A copy of the EX Approval Letter for the device and additional names, if any,
- Statement certifying that the firework devices to be shipped under the additional item name(s) provided continue to be produced by the same manufacturer and are the same as the approved device in the original approval.
- Email, fax, or send the Request with supporting documentation to the Office of Approvals.
