This report addresses barriers, emphasizes opportunities, and recommends guidelines for electronic information sharing between patients and clinicians. Specifically, in envisioning a world beyond the paper-based patient “chart,” it examines the role of patient-controlled personal health records (PHRs) and their interactions with clinician-controlled electronic health records. The report is a component of Phase II of Connecting for Health, building upon the Phase I work related to PHRs.
