The address of the laboratory covered by this recognition is: Communication Certification Laboratory, 1940 West Alexander St., Salt Lake City, Utah 84119. Recognition becomes effective 6/21/91 and will be valid until 6/21/96, unless terminated prior to that date, in accordance with 29 CFR 1910.7.
