  Where should I send my completed Application for a Social Security Card?

  I have completed my Application for a Social Security Card (SS-5).  Where do I send the application?

In most cases, you can mail or take your application and original documents to your local Social Security office.  If you live in the New York City metropolitan area, Las Vegas, Orlando, Phoenix or Sacramento you may need to visit your local Social Security Card Center in person.  First-time applicants age 12 or older also must apply in person.

People who live or receive mail in Brooklyn, N.Y.; Queens, N.Y.; Las Vegas, Nev.; Orlando Fla.;  Phoenix, Ariz.; or Sacramento, Calif.; must apply at one of the following Social Security Card Centers. These offices serve both U.S. citizens and noncitizens applying for new or replacement Social Security cards.

Brooklyn Social Security Card Center

Thursday, 7:00 a.m. to 7:00 p.m.

Queens Social Security Card Center

Wednesday, 7:00 a.m. to 7:00 p.m.

Las Vegas Social Security Card Center

If you live or receive your mail in Orange, Osceola, or Seminole Counties in Florida, and you wish to apply for a new or replacement Social Security number card, you must go to:

Residents of Sacramento County must visit the Social Security Card Center except for certain ZIP codes in the northeast and northwest corners of the county. Residents of 95610, 95611, 95621, 95628, 95630, 95662, 95671, 95763, and 95841-95843 may continue to conduct Social Security card business at the Roseville Social Security office. Those in 95833-95837 can use the West Sacramento office. All others must visit the Sacramento Card Center.
