Special Notice Concerning a New Requirement for Responsible Persons

A Special Notice will be included in all original and renewal Federal explosives application packages sent from the National Licensing Center. This notice explains the new requirement for Federal explosives licensee and permittee applicants to submit a Responsible Persons Questionnaire. The ATF F 5400.28, Responsible Persons Questionnaire, is currently unavailable. ATF F 5400.28, Responsible Persons Questionnaire, is currently only available to new Federal explosives license/permit applicants and to renewal applicants. Current Federal explosives licensees/permittees, whose license/permit expires after May 24, 2003, do not need to submit this form at this time. Additional instructions for those renewal applicants after May 24, 2003 will be made available at that time. This form will not be posted on the Internet for this reason.
