The Grants and Audit Management Branch (GAMB) is pleased to announce the availability of our new FSR PREP Package on the Region 3 Website. This new "post award" package contains useful files for any of our Region 3 assistance recipients who need to prepare and submit their Financial Status Reports (FSRs), in conjunction with grants and/or cooperative agreements they have already received.
The Prep Package files below are in PDF format. To ensure you will be able to see a PDF file in its entirety, please obtain the most recent edition of the free Acrobat Reader from the Adobe Web site.
The forms are available for download and use. Note that they are also 'writeable'. You may fill in the forms while you are viewing them and print the completed form. You may also save them to your local computer for future use.
If you have the full Adobe Acrobat software package, you will be able to save a copy of the form with your input. If you have downloaded only a copy of Acrobat Reader, you will be unable to save your input with the form.
- OMB Form 269A (10kb), the "short" form FSR most of our recipients will use;
- OMB Form 269 (88kb), the "long" form FSR for use by our recipients who are reporting Program Income on their FSR;
- "Preparation of Financial Status Reports (FSRs)" (1.5M, 17 pages), a Region 3 instructional document to help you prepare your FSR which includes specific instructions and examples;
- Air Program Maintenance of Effort (MOE) Worksheet (8kb), which should be prepared and submitted along with each Section 105 Air Program FSR;
- Non Point Source Program Maintenance of Effort (MOE) Worksheet (7kb), which should be prepared and submitted along with each Section 319 Non Point Source Program FSR;
- Radon Program CAP Cost Analysis Worksheet (6kb), which should be prepared and submitted with each Section 306 Radon Program FSR.
