REACH 2010 Releases Two
Success Stories Books
CDC’s Racial and Ethnic Approaches to Community Health (REACH)
program has produced significant improvements in reducing health
disparities in communities across our nation. REACH U.S. communities
will continue developing, implementing, and evaluating a broad range
of community-based strategies. The following documents were
developed to disseminate measurable health outcomes from select
REACH 2010 communities; and the population-specific strategies and
lessons learned can be used by all U.S. communities who are
interested in addressing health disparities.
The Power to Reduce Health Disparities: Voices from REACH
Communities

Through the use of evidence- and practice-based strategies, the
REACH 2010 communities found solutions and made significant advances
towards reducing health disparities and improving the health of
people in various settings, such as communities, schools, and work
sites. The 40 REACH 2010 Community Profiles in this book highlight
their successes and unique contribution in addressing health
disparities at the community level. Also view the

REACHing
Across the Divide: Finding Solutions to Health Disparities

REACH communities are innovators in developing strategies and
interventions that reduce health disparities among racial and ethnic
minority groups in the United States. The successes and lessons
learned speak powerfully of the importance of working with
communities to obtain optimal health for its members, including
engaging local leaders and organizations, forging community
partnerships, and recognizing cultural influence and historical
legacies. Putting into action the efforts and initiatives
highlighted in this book are essential to close health gaps and
allow all people to achieve the best health possible. The community
profiles are also featured in The Power to Reduce Health
Disparities: Voices from REACH Communities.
