Chronic Disease Prevention

The purpose of the program is to: prevent death and disability due to chronic diseases and promote healthy behaviors across the entire life span. The program accomplishes these goals in partnership with health and education agencies, major voluntary associations, the private sector and other federal agencies.

PERFORMING
Moderately Effective

In general, a program rated Moderately Effective has set ambitious goals and is well-managed. Moderately Effective programs likely need to improve their efficiency or address other problems in the programs' design or management in order to achieve better results.
  • The program has developed a challenging and ambitious set of long-term and annual outcome measures. The long-term and annual performance measures are based on program vision, mission, and positive health outcomes of the Nation.
  • The program has effective program management policies and procedures in place to ensure accountability of funds, efficiencies and cost effectiveness both internally and among grantees and partners. Grantee performance data are communicated to the public in a transparent and meaningful manner.
  • The program is making progress toward achieving its long-term and annual performance goals and is continuing to show success over time. For example, rates of cigarette consumption by adults decreased by an average of 4 percent annually in the years prior to the establishment of the new baseline for this goal. Decreases in cigarette consumption decrease cancer rates.
