I think these forms are great. It would be nice to have them in slightly larger print. I also think the broker should have to disclose nuisance fees like the annual fees charged on most IRAs and Annuity Accounts.
On a separate issue, it would be nice to have the Prospectus show how an investment would change if the dividends are not reinvested.
