Qualitative and quantitative evaluation of the antibodies and immune response after pneumococcal and Haemophilus influenzae vaccines in HIV-infected children.

Backgrounds: There are few studies evaluating IgG subclasses levels in HIV infected children and the response to polysacharides vaccines. Severely immunocompromised patients do not respond well to the routine immunization. The objective of the study was to evaluate these aspects in a cohort of Brazilian HIV-infected children. METHODS: Group A: 33 children perinatally HIV-infected, Group B: 130 uninfected children (controls) were included for the measurements of antibody response, antibody avidity to H. influenzae (Hib) and viral load after vaccination. RESULTS: Most of HIV-infected children presented hypergammaglobulinemia but 30% and 21% showed reduced levels of IgG2 and IgG4, respectively. The antibody responses to Hib and to 23-pneumococcal vaccines were poorest in HIV-infected children when compared to healthy controls. It was not observed statistical diference between Group A and Group B about the avidity to Hib antigen after immunization. The authors did not find description of this latter aspect in the revised literature. The HIV-1 viral load became stable after the administration of both vaccines. CONCLUSIONS: Despite the poorer response to both vaccines, more proeminent with the 23-valent pneumococcal vacccine, these data suggest that HIV-infected children should be immunized with polysaccharide-conjugated vaccines and that the use of inactivated vaccines didn't provocate a significant increase in the replication rate of HIV infection.
