Quality of care in STD/AIDS health services in Sao Paulo, Brazil: a qualitative framework.

BACKGROUND: In 1999, an evaluation on HAAART adherence in 27 health services in Sao Paulo State, Brazil showed that the health provided care is the main factor related to the adherence. This conclusion was the basis of this study now focused on quality of care. METHODS: interviews with the managers, observation of work and structured questionnaire based on the quality criteria of integration of process of care. The criteria was analyzed through the indicators: prioritization of important work targets; use of different and specific technical approaches; evaluation activities; team integration, and communication between professionals and patients. RESULTS: The content analysis has shown that the indicators can be assessed through the empirical categories: routine for the patients who miss appointments and/or interrupted the treatment, are pregnant or drug users and who have their first contact with the service, routine discussing sexuality, conception, and adherence; regular use of psychological and social approaches; regular mechanisms of integration among different professionals, e.g. meetings, case discussions and training; initiatives in enhancing dialogue with patients, e.g. discussion groups and with the community, e.g. ombudsman and cooperation with NGO; skills coping with non predictable situations, e.g. patients who frequently miss appointments or refuse the treatment or ask for other kinds of appointment routines; manager leadership in dealing with technical and ethical issues on team interaction. With the analysis of this factors network it was possible classified the 27 services in 3 levels of quality. CONCLUSIONS: The framework was able to evaluate the quality of care according to the integration of process of care. The results are being used to define norms for the quality of care evaluation that is being carried out in the whole AIDS Brazilian Program.
