Quality of care in short term geriatric units: salient criterias identified by patients and caregivers.

RESEARCH OBJECTIVE: To identify criterias that patients and caregivers refer to when they appreciate the quality of care of Short Term Geriatric Units (STGU). STUDY DESIGN: Face-to-face semi-structured interviews were conducted with 21 patients recently released from 6 STGU in Montreal. Four Focus Groups were conducted with caregivers. Interviews were constructed in order to cover positive and negative aspects of the care received throughout patients' hospitalizations. Content analysis was based on transcriptions of tape-recorded interviews. Coding of the data was done independently by two members of the research team and then revised in order to achieve consensus. PRINCIPAL FINDINGS: Patients and caregivers reported respectively 79 and 42 different quality criterias. Caregivers' criterias referred to patient and family care. Both groups had expectations regarding notably the technical competence and interpersonal manner of doctors and nurses, the atmosphere on the ward and the information provided. CONCLUSIONS: Some criterias used by elderly are similar to those reported by patients in various acute care units and other criterias are specific to the frailty of their condition. Both groups tend to use different criterias to assess the same dimensions of quality. RELEVANCE TO CLINICAL PRACTICE AND POLICY: This study provides health care professionals with rich information on how patients and caregivers perceive the quality of care in STGU and content material for the development of two separate satisfaction tools.
