OBJECTIVE: To broaden the concept of "quality of care" in light of the evolution of new and integrative practice patterns in the AIDS Designated Centers in New York State in order to encompass standards for case management, informed consent, right of patients to receive treatment, and attention to needs of the patients' family and/or support persons. METHODS: Chart review, conducted for both concurrent and retrospective cases, includes assessment of the role of the multi-disciplinary treatment team, the comprehensive care plan, the patients' response to treatment, nursing care, and documentation of specialty consults and patient consent. Another innovative aspect of this enhanced review process is the effort invested in constructive feedback to the provider which includes rendering advice, assistance, or technical information to hospital staff. RESULTS: The impact of the review process may be demonstrated through a reduction in the proportion of reviewed cases with quality of care problems over the first three site visits. For eleven hospitals reviewed between April 1987 and June 1989, the first visit found 55.8% of cases with quality "issues", the second visit yielded 38.1% of cases with quality issues, while the third visit, saw a reduction in the proportion of cases with problems down to 11.7%. CONCLUSION: The enhanced review process appears to lead to a reduction in the proportion of cases with less than optimal quality of care.
