Form LM-3, Labor Organization Annual Report
The Form LM-3, Labor Organization Annual Report, discloses financial information about labor organizations, such as assets, liabilities, receipts and disbursements, which have total annual receipts of $10,000 or more, but less than $250,000.
Labor organizations can acquire and complete Form LM-3 in several different ways:
1. Download the Electronically Fillable PDF Form – This is the fastest, most accurate way to complete and file Form LM-3. A Download feature accesses your labor organization’s identifying information from our database and enters it on your form. You can then save the form and complete it on your computer. The software performs calculations for you, and runs an error check. It also permits unions that keep records electronically to import financial data from their accounting programs into the electronic form.
The fastest, safest, most efficient way to file the form with OLMS is to sign it with digital signatures and file it electronically. You may also choose to print a hard copy of the completed form for your officers to sign manually and mail the completed signed form to OLMS at the address given in the Form Instructions.
2. Download and Print a Non-Interactive PDF Form – You can download and print out a non-interactive version of the Form LM-3 PDF, complete it manually, and mail the completed signed form to OLMS. Note that this version of the form does not pre-fill your labor organization’s identifying information, perform calculations for you, or run an error check.
3. Contact OLMS for a Hardcopy Form LM-3 – If you are unable to download or use either PDF version of Form LM-3, contact OLMS to order paper copies of the form and form instructions:
