There are three methods that can be used to prepare and submit your Strategy. Choose the method that is most convenient for you.
- Strategy Form (PDF, 3 pp., 405 KB, about PDF) - Using Adobe Acrobat Reader, enter your information, print the completed form, and mail a copy to:
- Electronic
Strategy Form (HTML) - After entering your information on screen,
print a copy, and click on the "Submit"
button to automatically email your strategy to us.
- Lastly, you may fill in the blank forms mailed to you and return the completed form to the address above.
